    Surgical treatment in children missed Monteggia lesions at Bhuj, Kutch, Gujarat: a retrospective study

    Background: The treatment of an unrecognized Monteggia lesion continues to pose a therapeutic challenge, as evidenced by the variety of surgical techniques described. Moreover, there are high complication and redislocation rates following surgery. This report concerns a surgical technique to reduce a chronic dislocation of the radial head utilizing an ulnar osteotomy and internal fixation.Methods: Present study was performed at department of orthopedics, Gujarat Adani institute of medical science, Bhuj, Kutch, Gujarat. Ethical clearance was taken from the institutional ethics board and informed consent was obtained from all the participants. Between July 2015 and September 2016 six children presented in a traumatic context with chronic dislocation of the radial head and malunion of the ulna.  Patient mean age was 6.5 (range 4–8) years, and the mean interval between injury and surgical procedure was 17 (range 1–49) months. Surgery consisted of an ulnar osteotomy with angulation and lengthening, bone grafting at the osteotomy site, and internal fixation. Open reduction of the radial head, repair or reconstruction of the annular ligament or temporary fixation of the radial head with a transarticular wire was not undertaken. Cast immobilization with the forearm in neutral rotation was maintained for 4-6 weeks.Results: There was one case of nonunion. At an average follow-up of 3 (range 1.5–4.4) years, all patients had regained painless function of the forearm, good range of elbow and forearm motion, and maintenance of the radial head reduction. Conclusions: Both angulation and elongation of the ulna are required to allow for reduction of the radial head. We do not see any indication for procedures directed at the radio-capitellar joint

    Management of congenital vertical talus: comparison between mini invasive reduction and extensive surgical technique in early age

    Background: About one baby in 10,000 is born with the foot deformity known as vertical talus. In about half of those cases, both feet are affected. The disorder occurs with equal frequency in boys and girls. Vertical talus is a rare deformity of the foot which is diagnosed at the time of birth.Methods: Present study was performed at Department of Orthopedics, Gujarat Adani Institute of Medical Science, Bhuj, Kutch, Gujarat. In the study 40 patients who were treated for unilateral or bilateral congenital vertical talus at a single institution which lead to 60 treated feet in the study. Patient with bilateral symptoms were randomly selected for one foot in the study.Results: The mean normative pain score was found to be 58.0 in the minimally invasive group compared with 28 in the extensive release group (p =0.032). There was no difference between the two groups in the transfer and basic mobility domain. The normative global function domain was higher in the minimally invasive group (48.3) compared with the extensive soft-tissue release group (34.3, p =0.03). Conclusions: Longer-term follow up and studies with large sample size are still necessary to determine whether the improved outcomes are maintained into the adulthood and whether the superior outcome is related to the reduce scarring in the patient

This thesis present the result of stress analysis done on a crankshaft of a single acting Four Stroke Diesel Engine using Solid Work, ANSYS and FEMAP Software. The three dimension model of crankshaft are develop in Solid work software and analysis in solid work first and after that import in ANSYS and FEMAP for stress analysis. The static analysis is done and is verified by simulation in finite element analysis software Solid work, ANSYS and FEMAP .A calculation method is used to validate the model. Finite element analysis is performed to obtain the variation of stress magnitude at critical locations

    Atmospheric correction using 1240 and 2130 nm combination of MODIS SWIR channels

    It is essential to improve understanding of coastal ocean since the majority of the world's primary production occurs on continental shelves and the coastal ocean is most utilized and impacted by humans. The first step in ocean-colour data processing is the removal of atmospheric contribution from the sensor-detected radiance to enable detection of optically active oceanic constituents e.g. chlorophyll-a, suspended sediment etc. Black ocean assumption at the near infrared (NIR) wavelengths as applied to perform atmospheric correction fails for coastal turbid waters due to the presence of highly scattering sediments which cause sufficient water-leaving radiance in NIR wavelengths and lead to over-estimation of aerosol radiance for λ <700nm resulting in negative water leaving radiance for λ <500nm. The assumption of zero water-leaving radiance at the NIR wavelengths was replaced by the assumption of zero water-leaving radiance at the short wave infrared (SWIR) wavelengths over the coastal turbid waters and atmospheric correction was performed using these SWIR wavelengths. Physically realistic and positive water leaving radiances throughout the spectrum and especially for shorter wavelengths (412nm, 443nm, 490nm) were obtained over coastal turbid waters using this concept

    Cell Permeabilisation, Microstructure and Quality of Dehydrated Apple Slices Treated with Pulsed Electric Field During Blanching

    ABSTRACTEffect of pulsed electric field as a blanching pretreatment on cell permeabilization, microstructure and quality of dehydrated apple slices was studied. Apple slices were pulsed electric field pretreated (1.0, 1.5 and 2.0 kV/cm using 25 and 75 pulses) at a temperature of 60 and 80°C in water followed by dehydration at 60°C in a cabinet dryer. Cell disintegration index was found to increase significantly (p<0.05) with increase in the electric field strength, number of pulses and blanching temperature and thereby reducing the drying time. The dehydrated slices showed lower hardness and higher lightness (L*) values for the samples treated at higher electric field strength for longer durations. Scanning electron microscopic studies of samples revealed better retention of cellular integrity when pre-treated at a low level of PEF (1.0 kV/cm using 25 pulses) and blanched at low temperature (60°C). However, the samples treated to a level of 1.5 kV/cm and 75 pulses of PEF with subsequent blanching temperature 80°C was found to yield optimum cell permeabilization. The study suggests that PEF can be used as an effective blanching pretreatment for achieve good quality dehydrated apple slices in less drying time.
